About Apple Community Health Services

Apple Community Health Services (ACHS) is a case management agency approved by the Georgia Department of Community Health to provide Traditional and Enhanced EDWP (Elderly and Disabled Waiver Program) case management, located at 1708 Peachtree Street NW, Suite 410, Atlanta, Georgia, serving metro Atlanta.

What a case management agency does

Georgia's Elderly and Disabled Waiver Program pays for care at home for people who would otherwise qualify for a nursing facility. Somebody has to work out what that person needs, write it into a care plan, arrange the providers who deliver it, and keep checking that it is actually happening. That is case management, and it is what we do.

We are not a home care agency. We do not send the helper — we arrange for a provider agency to send one, and then we monitor the plan. The difference between the two is the single most common confusion families arrive with.

Medicaid pays the case management agency directly. No bill for case management ever reaches the member or the family.

Two things that are unusual here

The Georgia case management manual (§2038) requires the Lead Case Manager to be a Georgia-licensed registered nurse. Ours is Cathy Nisbett, RN, CCM, with more than twenty-five years in the field.

General Services §602.11 allows either the member's own physician or the case management agency's medical director to approve the plan of care and attest that the member needs a nursing-home level of care. We have a medical director on staff, which means that step does not depend on getting time with the member's own doctor.

You choose your agency

Under §1894 a member has the right to choose their Traditional or Enhanced EDWP provider, and under §1896 a case manager is obliged to tell you what agencies serve your area and let you choose among them. You can also change agencies later. We publish that right in full, including how to complain about us.
