Not live yet. A referral form carries health information about an identifiable person, so it will run on a vendor that has signed a Business Associate Agreement and holds the data on their side — never on this site's own infrastructure.

Do not send member health information by ordinary email. It is not a secure channel and we will ask you to resend it another way.
What we need from you
A referral can start without a complete packet. Gaps cost time rather than stopping the referral — each line below says what it is for.
- Who the member is
- Name, date of birth, and a phone number that reaches them or the family contact. Without a reachable contact we cannot start.
- Medicaid status
- Medicaid number if they have one, or where the application stands if they do not.
- The clinical picture
- Diagnoses, current level of function, and what help they need on an ordinary day.
- Level-of-care documentation
- Anything you already hold toward the DON-R or the DMA-6. If you have none, say so — it does not stop the referral.
- The transition context
- Discharge date if there is one, where they are going, and who is covering the gap.
- Consent and authority
- Who has authority to speak for the member, and confirmation that they know a referral is being made.
The member chooses their agency
Under §1894 a member has the right to choose their Traditional or Enhanced EDWP provider, and §1896 obliges a case manager to tell them what agencies serve their area. Present ACHS as one option among those that serve the member's county. We would rather be chosen than assigned.
